After completing the "Personal Information" section in the Questionnaire, use the "Proposed Beneficiaries of Your Estate" section to enter in the names of those persons that you want to receive EQUAL shares – whether only a portion or all – of your estate.  Notice that the share entry field in this section contains read-only text showing the word "equal" – that cannot be edited.  Again, that is because you are to enter there only the names of those persons who are to receive EQUAL shares of a certain portion or all of your estate.
